To refer a student to Community Council Learning Academy please fill out the form below.

This form must be submitted by an administrator in a school district; we will not accept submissions by parents.

The information submitted in this Referral Form is only shared with the CCLA administrators.

Please note: Your referral isn’t finalized until all SPED documents are sent to intakeccla@cchss.org (IEP, PBSP, FBA, RER). If accepted, more documentation will be required during the actual intake.
